disks with no reported errors show as DEGRADED with 'too many errors'

Description

This is on a brand new installation of 3.0 RC3 on an Adaptec ASR-2405. I'm not sure what information is going to be useful so please let me know, but for now, here's the basic zpool show:

pool: backup state: DEGRADED status: One or more devices are faulted in response to persistent errors. Sufficient replicas exist for the pool to continue functioning in a degraded state. action: Replace the faulted device, or use 'zpool clear' to mark the device repaired. scan: none requested config:
